Fees:
Full payment $1300 is due at the time of service. If a payment plan is needed, $650 is due to hold the appointment. The other half $650 is due at the time of the appointment. Credit cards will not be charged without permission and appropriate documentation/consent forms will be required in order to secure your appointment.
In the event that your evaluations need to be expedited, there is an additional $300 required in addition to the base fee.
